New Pediatric Patients

If you are interested in joining our practice, there are a few steps that you must complete first. You must call your insurance company to change your primary care doctor to one that you have chosen from our practice.  

You will also need to fill out a medical release form in order to have your child's medical records transferred.  This is an imporant step but is not necessary for your first appointment.

Parents must bring in a valid photo ID, a copy of their child's Insurance cards and any medical records / immunization records you may have on hand to your first appointment.

Newborn Appointments

If you are expecting a newborn and plan to deliver at Signature Healthcare Brockton Hospital, one of our doctors will be available to see your newborn in the hospital. Hospital personnel are also able to call the office of your choice to book your newborn their first visit.

If you plan to deliver at any other doctor, give us a call once the baby has been born to book your newborn their first appointment! Let the hospital know you have chosen us as your newborn's pediatricians so that they may forward any hospital records we may need.

We like to see newborns within two days of being discharged from the hospital.

We ask that parents bring in a photo ID, their insurance cards and any hospital records they may have to their first newborn appointment.
